Please follow three easy steps below to complete your new hire paperwork.

Step 1: Form I-9, Employment Eligibility Verification

Print and compete this form.

* Please select one form of identification from list A or one form from both B and C and bring it with you on your first day of employment (see page 5 of the form for list). The I-9 will be completed at that time.

Step 2: State Tax Form

Click this link and select the state in which you will be working. If you are working remotely choose the state in which your home office resides. Print and complete the form(s).

* If you are an Ohio resident working in the Covington, KY office, you will also need to select the certificate of non-residence Kentucky form. If you are a New Jersey resident, working in the New York, NY office, you will also need to select the certificate of non-residence New York form.

* Temporary employees can disregard.

Step 3: New Hire Paperwork

Select the appropriate packet below based on your employment status. Please note there are separate packets for California residents. Each packet includes instructions on which forms to print and complete.

Attention Recent Graduates

If your background check was completed prior to your graduation date, please bring proof of your completed degree along with your new hire paperwork on your first day. Acceptable documents include a copy of your diploma or a copy of your completed transcripts.
