To complete the online Health Assessment Questionnaire:
- Go to the StayWell secure website.
- If you are a returning user, you'll be directed to create a new password. Just follow the online instructions.
- If you have never logged in before, your first time password is nielsen123. To ensure confidentiality, you will need to change your password after you log in for the first time.
- Click on Take Your Health Assessment.
- Make sure you select Finish & View Results to submit your answers and receive your immediate results. Select Save & Finish Later only if you intend to come back at a later date to finish.
